Major responsibilities include:Works independently in managing a wide scope of cases including more complex medicalCompiles patient history, performs necessary examinations, orders appropriate diagnostic tests, formulates diagnosis, and prescribes appropriateHandles inbound calls from patients directly or calls transferred from case management nurse/patient advocate.Perform periodic outbound calls to ensure that patients are directed to and receive care in appropriate venue in a timely manner.Provide remote treatment when/if condition could be managed in house while following TruDoc 24X7 policies and procedures.May offer in-home assessment to patients with identified barriers to appropriate healthcare facilities.Remotely monitor enrolled patients through telemonitoring software.Enrolls patients in Chronic Care program and perform complete assessments for identified patients who may benefit from this program.Manages common health problems, emergencies, chronic diseases, and acuteProvides health promotion and preventive services including: child health services, ante natal care, screening, and pre-marital counseling.
